WHAT IS SHOCKWAVE THERAPY?

Shockwave therapy - sometimes called Extracorporeal Shockwave Therapy (ESWT) or Radial Shockwave Therapy (RSWT) - is a non-invasive, drug-free treatment that uses high-energy acoustic pressure waves to stimulate healing in injured tissues. The shockwaves travel through the skin into the affected tendon, fascia or muscle and trigger a cascade of biological responses: increased blood flow, stimulation of collagen production, breakdown of calcified tissue, and a re-set of the local pain signalling.

Unlike pain relief medication, which simply masks symptoms, shockwave therapy targets the cause and supports the body to heal itself. It is approved and used worldwide as a first-line treatment for many tendon, fascia and chronic soft-tissue conditions, and is included in international clinical guidelines for several common musculoskeletal injuries.

HOW DOES A SHOCKWAVE SESSION WORK?

Your osteopath positions the shockwave hand-piece directly over the area of pain or injury. The device delivers rapid, focused acoustic pulses - typically a few thousand over a 5–10 minute treatment - with the intensity adjusted to your tolerance and the depth of the tissue being treated.

Most people describe the sensation as a strong tapping or pressure. It can feel intense at times, but it remains controlled and brief. Treatment is short (usually under 15 minutes), requires no anaesthetic, and there’s no downtime - you can walk out and carry on with your day.

At a tissue level, these pulses stimulate nerve fibres (C fibres), prompting the release of a neurochemical called substance P. This can contribute to the temporary discomfort felt during or shortly after treatment. However, with repeated stimulation, these fibres reduce their ability to release substance P, which helps decrease pain over time.

Lower levels of substance P also reduce neurogenic inflammation - one of the drivers of persistent pain. Alongside this, shockwave therapy supports the release of growth factors and may stimulate cellular repair processes within the tissue, creating an environment that encourages healing and recovery.

Shockwave therapy is brilliant - but only when it's the right tool for the job. Before we book a course of treatment, we ask new patients to come in for a focused clinical assessment with one of our osteopaths.

 

This visit lets us:

  • Confirm a clear diagnosis - many conditions present similarly but need different treatment.

  • Rule out contraindications such as recent fractures, blood-clotting issues, infections, or pregnancy.

  • Decide whether shockwave alone is best, or whether it should sit inside a wider rehabilitation plan with osteopathy, exercise prescription or Pilates.

  • Discuss what you can realistically expect, how many sessions you may need, and how to support recovery between visits.
